What ReLEx SMILE is
ما هو؟
ReLEx SMILE (Small Incision Lenticule Extraction) is a third-generation laser vision correction. Instead of creating a corneal flap as LASIK does, the femtosecond laser carves a thin disc-shaped lenticule inside the cornea. The surgeon then removes that lenticule through a 2 mm keyhole incision. Removing the lenticule reshapes the cornea to correct your refractive error.

Because there is no flap, more of the cornea's outer biomechanical strength is preserved. The 2 mm incision is much smaller than a LASIK flap edge, which is why SMILE is associated with less post-operative dry eye and faster corneal stability.

More people are SMILE candidates than LASIK candidates because the technique is gentler on the cornea.

- Myopia up to -10 dpt, with or without astigmatism up to 5 dpt

Every surgery has risks. Here they are honestly.
ReLEx SMILE on the Carl Zeiss VisuMax is a flapless procedure with a 2 mm incision — a smaller surface footprint than LASIK. That changes the risk profile, it does not eliminate it. The candidacy consultation is when these are discussed in writing.

- Dry eye in the first 1–3 months — usually milder than LASIK because no flap is created.
- Halos at night for the first weeks; fades for most patients within 1–3 months.
- Mild visual blur or fluctuation as the cornea heals (24-hour cornea heal, 48-hour return-to-work for most patients).
- Suction sensation reported during the laser portion (~32 seconds per eye).

- Difficulty extracting the lenticule on the day of surgery — uncommon; managed by the surgeon as it occurs.
- Suction loss during laser cutting — very rare; treatment can typically be re-attempted or converted to LASIK.
- Under-correction needing enhancement — less common in SMILE than LASIK because of the precise lenticule cut.
- Infection (endophthalmitis) — very rare; single-use sterile instruments and prophylactic antibiotics.
- Ectasia (corneal weakening) — rare; the workup screens for this.
SMILE is currently FDA-approved for myopia and astigmatism within specified ranges (myopia up to ~-10 dpt, astigmatism up to ~5 dpt). Outside those ranges, alternatives are considered. Your specific candidacy is documented before surgery.

LASIK creates a hinged corneal flap, lifts it, reshapes the underlying cornea with a second laser, then lays the flap back. SMILE removes a thin disc-shaped piece of the cornea through a 2 mm incision, no flap. The result is similar; the technique is gentler on the corneal surface and is associated with less post-operative dry eye.

No. Topical anesthetic eye drops are used. Patients describe the experience as "weird" rather than painful — there is mild pressure on the eye for a few seconds. To treat both eyes from the moment drops are applied to leaving the laser suite is 15–25 minutes total.

The cornea heals completely within about 24 hours since the incision is only 2 mm. Most patients drive the next day, return to office work in 48 hours, and resume swimming and sports after one week.

SMILE primarily treats myopia (short-sightedness) up to -10 diopters and astigmatism up to 5 diopters. For pure hyperopia (farsightedness), Ultra LASIK or other procedures may be more appropriate. The consultation will determine the right fit.

The corneal reshaping is permanent. Age-related changes (presbyopia from the mid-40s, cataract later in life) happen in everyone and are independent of refractive surgery.

Generally yes. The flap edge in LASIK interrupts more of the corneal nerve plexus that signals tear production, so post-LASIK dry eye is more common and more pronounced. SMILE's 2 mm incision interrupts much less of that nerve plexus. Patients with pre-existing dry eye are more often offered SMILE.
